Judge Partially Blocks Medicaid Frailty Work-Exemption Rule

Federal judge partially blocks CMS guidance requiring Medicaid recipients in medically frail categories to prove conditions impair work compliance; work rules stand.

THE DECISION

The story in focus

A federal judge in Boston partially blocked Trump administration Medicaid guidance requiring medically frail recipients to show their conditions significantly impair compliance with work requirements, ABC News reported. The ruling sides partly with 25 states and D.C. that sued in June; it leaves the underlying work requirements intact before their Jan. 1 start.

  • U.S. District Judge Richard Stearns issued the ruling in Boston, according to ABC News.
  • The coalition of 25 states and the District of Columbia sued in June.
  • The guidance tied to the 2025 law required frailty categories to show significant impairment of work compliance.
  • Work requirements begin Jan. 1 for expansion enrollees aged 19 to 64, with exceptions.
  • Stearns upheld limits rejecting diagnoses older than a year and allowing one self-attestation per enrollment period.

What the judge decided

U.S. District Judge Richard Stearns in Boston partially blocked Trump administration CMS guidance that required Medicaid recipients in medically frail categories to additionally show their conditions significantly impair their ability to meet work requirements, ABC News reported. The ruling gave a partial win to a coalition of 25 states and the District of Columbia that sued in June.

Stearns said the states showed irreparable harm, citing the burden of costly alterations to eligibility verification systems, according to ABC News. The decision does not strike down the work requirements as unlawful.

What remains in place

Starting Jan. 1, expansion enrollees aged 19 to 64 must work or do community service at least 80 hours a month or attend school at least half-time, with exceptions including medical frailty and addiction treatment.

Stearns upheld two verification restrictions: rejecting frailty proof when the diagnosis is more than a year old, and allowing only one self-attestation per continuous enrollment period. He said the government gave a non-frivolous rationale for each provision.

Next steps

HHS and CMS representatives did not immediately respond to a request for comment, ABC News reported. The ruling applies as the work requirements approach their scheduled Jan. 1 effective date.

Does the ruling cancel Medicaid work requirements?

No. ABC News reported that Judge Stearns did not strike down the work requirements, which take effect Jan. 1. The ruling eases the medical-frailty exemption path while leaving verification limits intact.
